Where is Advanced Ob/Gyne located?
We have 2 offices to serve you located in Hoffman Estates and Bloomingdale.
You can find a map of our location in out location/hours link on the left side
of this web page.
Where should I park?
There is ample parking at both the Hoffman Estates and Bloomingdale locations.
If you're appointment is scheduled in Hoffman Estates, you will want to park in
the North Parking lot adjacent to Doctors Building II.
Should I fill out and bring the patient forms with me to my next appointment?
While it is not required to have your patient forms completed prior to your
appointment arrival, it would help to expedite the patient check in process.
We
have provided most of the forms you might need in our patient forms
section of
the web site. You can access the forms which are in pdf format through that
page. Simply fill them out and bring them with you at your appointment time.
I am a new patient what should I bring with to my first appointment?
Please bring with you a photo I.D. and any proof of insurance, such as your member card. You
can also fill out, print and bring any new patient forms
from our web site prior
to your appointment. which will help to expedite
check in upon your arrival.
What insurance do you accept?
We accept most major insurance plans, you can find a detailed listing in our
insurance section of our web site.
What are your hours?
Our hours office hours are located in the Location/Hours section of our web site. If you should need medical attention when our office is closed please contact us at 847-755-111. If it is a life threatening situation, please call 911.
